Forms and Documents Associated With Contracts
- Payee Data Record, Std. 204, Revised August 2006. This State Department of Finance form is used by CDIAC to prepare information returns (Form 1099). The contractor must submit a completed form to CDIAC in order to receive payment from the State of California.
- Small Business/Disabled Veteran Business Enterprise Participation Summary. These forms must be filed with the Office of Small Business Certification and Resources, not CDIAC.
- Contract Certification Clauses (CCC). The contractor must sign and submit CCC to CDIAC for all contracts and proposals such as IFBs and RFPs.
